According to federal law, employers no longer have to automatically send Form 1095-C to individuals. Should you require a 1095-C, please email a request to Katie Maslanka at kmaslanka@maderconstruct.com or call the office at 716-655-3400 and ask for Katie Maslanka at extension 222. Upon your request, we will send you a 1095-C form.
